Maternity and Leave Information
Congratulations - you’ve just learned you’re going to have a baby! Now what? Here is some information to help you:
It is your responsibility to:
- Notify your Principal and/or Office Manager
- Notify Human Resources
Please notify Ellen Bosse in Human Resources @ 432-2115 as soon as possible so that you can obtain the appropriate forms for completion by your site administrator and your doctor or download the forms by clicking on the appropriate link below:
- Verification of Maternity Confinement and Ability to Return to Work PDF
- Unpaid Leave of Absence Request PDF
- The Standard or American Fidelity coverage - Call Human Resources
You’ve Had Your Baby - Now What??!!
- Call Ellen Bosse @ 432-2115 to inform her of your delivery date.
- Call Cathie Swoboda @ 432-2118 to inform her of the birth of your baby for insurance purposes.
Pregnancy Disability?? Maternity Leave??
Family Medical Leave??
Personal Business Leave??
